Fall Athletic Registration
The registration dates are from August 2nd thru August 10th. Forms can be turned in from 9:00am – 1:00 & 2:00 – 3:30pm at the Athletic Office.
If you are coming in for make-up registration on August 11th, you may submit your Athletic forms then.
Please print out and submit the following forms:
Athletic Registration Cover Page
Athletic Registration Packet (English)
Athletic Registration Packet (Spanish)
Submit complete with signatures, your $185 fee to FHS, and a current physical information. If you do not have access to a printer, please come to the FHS Library to print out your forms. We do not have Athletic Registration forms in the Athletic Office.
