Volunteer Forms and Information

  • Volunteer Background Screening Criteria
  • Permission to Transport Students in Private Vehicles - Required for all volunteer drivers, current proof of insurance coverage must be submitted with this form.
  • Complete an annual online background screening at - TCLogiQ accepts online payment (MasterCard, Visa, Discover, and American Express).
  • CBI Background Screening Information - CBI/Fingerprint background clearance precludes a volunteer from needing any further screening.
    • Step 1 - Get your fingerprints taken.  One fingerprinting location is the Boulder County Sheriff's Office, 5600 Flatiron Pkwy in Boulder, (303) 441-3600.  For Boulder County Residents, there is a $10.00 fee; for non-Boulder County residents, the fee is $15.00 (CASH OR CHECK ONLY).
    • Step 2 - Take your fingerprinting card, completed CBI Background Screening Information Sheet, and check for $39.50 made payable to ‘Boulder Valley School District’ to the BVSD Education Center, 6500 East Arapahoe in Boulder, and deliver all materials to the Office of Legal Counsel.
    • Step 3 - Please make a copy of your CBI Background Screening Information Sheet for the Mesa Office so the school knows that you have completed the CBI application process.

What are the dates and times of the early release for 2013-2014?
We will dismiss students at 1:20pm on the following Thursday dates: September 26th, October 24th, November 14th, December 19th, January 30th, February 27th, March 13th, April 17th.
What do students do at 1:20pm on an early release day?
There are three options: (1) students may go home with a parent/guardian as arranged, (2) students may stay at school to participate in a free, supervised extra-curricular activity provided by the Mesa para-educator support staff until 3:00pm, or (3) students may remain at Mesa and take a tuition-based YMCA class until 3:00pm (a parent/guardian must complete and return the separate YMCA registration form).
*The school busses, the YMCA after care program, and all other after-school programs will run at 3:00pm on an early release day, just like any other day (i.e., no bus at 1:20pm, no YMCA at 1:20pm).
What is the Early Release schedule?
School Arrival/Dismissal Schedule for Early Release: 8:25am - 1:20pm
The lunch schedule for grades 1–5 will be the same as other school days.
AM Kindergarten/KE Schedule:   8:25am - 10:40am*
PM Kindergarten/KE Schedule:   11:10am - 1:20pm*
*10:40am - 11:10am for Kindergarten lunch and recess
