Enrollment and Registration Forms  

We have an option this year to complete your paperwork electronically and e-mail them back to the school. All fees can be paid for on-line as well using our Pay-For-It program. You will need to have an Infinite Campus account set up in order to take advantage of this option. Please see the link below for information on setting up your account. The office will be open from 10-12 noon on August 12th to drop off paperwork and pay for fees. Please do not hesitate to contact the office staff if you have any questions regarding our required forms, fees or policies.

Enrollment Steps for New Students

Below are the forms, information and links to enroll your new or returning student at Douglass Elementary School. If you are not sure that your address is in the Douglass Elementary attendance area please check our School Finder. If Douglass Elementary is not your 'home school' you will need to apply through Open Enrollment process first.

Incoming Kindergarten Students

Children entering Kindergarten must be 5 years old on or before September 30th of the school year. Kindergarten hours are Monday/Wednesday from 8:00 a.m. to 2:30 p.m. or Tuesday/Thursday from 8:00 am to 2:30 pm. In addition there is a Friday AM from 8:00 a.m. to 10:45 am for Monday/Wednesday classes and Friday PM from 11:45 am to 2:30 pm for Tuesday/Thursday classes.

Douglass Elementary offers a fee-based enrichment program called Kindergarten Enrichment. Students are registered in Kindergarten Enrichment ONLY upon receipt of all forms below.

Step One: Complete the District On-Line Enrollment at Please contact the Student Enrollment Center if you have questions, 720-561-5247 or enrollment.helpdesk@bvsd.org

Step Two: After completion of the on-line enrollment process please print off:

• The printed PDF receipt/confirmation with the barcode
• Emergency Card 
• Home Language Survey

Step Three: Bring the above documents with following documentation and forms to Douglass Elementary School Office, 840 75th Street, Boulder, CO 80303, Phone 720-561-5541, Fax 720-561-6699.

• Copy of student's birth certificate or passport.
• Copy of student's immunization record. (see the School House Letter below for requirements)
• Proof of residency (copy of lease or utility bill such as Excel, Water or Garbage Disposal)
