Program Fees and Information
One of the major components of an excellent middle school program is to develop well-rounded students. In order to help accomplish this goal, we strongly encourage students to participate in the interscholastic and intramural programs offered here at Louisville Middle School. Check out the link below for a listing of athletics offered this year. Also, there are postings in the lunch room and big gym.
Prior to the first day of practice all students participating in INTERSCHOLASTIC athletics will be required to:
Obtain a current athletic physical. (A Sports Physical Health form is available from your physician and is good for one calendar year from the date of the physical examination.)
Complete an Interscholastic Athletic Packet Parent Permission/Athletic Eligibility Form (one per school year)
Pay a $85 Participation Fee
Prior to the first day of parctice, all students participating in INTRAMURAL athletics will be required to:
No student will be denied access to an activity based on fiscal need. Applications for fee waiver (use this link) or are available in the Main Office. In order to make your time at Louisville Middle School a full and rich experience, we encourage you to take advantage of the athletics that we have to offer.
