ATTENTION SENIORS
Please be sure we have up-to-date immunization records for inclusion with your college enrollment.  You will receive an Immunization Record when you receive your caps/gowns for graduation, so please submit any new immunizations to me by April 30, to have the most current record to submit to your college.
  
RULES FOR IMMUNIZATION & MEDICATIONS
Please click on the Key Link "Health Information necessary for all BVSD Students" for important information concerning immunization requirements and medications in school.  Any student needing medication while at school must have a "BVSD Medication Release Form" on file in the Health Room, signed by both guardian and physician.  All medications must be locked up in the health room, except if a physician has given permission for a student to self-carry an inhaler or epi pen.
 
EMERGENCY CONTACT INFORMATION
In the event of an emergency, it is imperative that the school has current contact information for your student.  Please be sure to complete this form and return to the school prior to the first day of school each year.  If your information changes during the school year, please be sure to let the office know ASAP.  Also, please list multiple contacts in the event we cannot reach the main contact.  An Emergency Contact Form is available to download in "pdf" format from the "Documents/Handouts" section of this webpage (see  column on left).
