Required forms (to print, sign and turn in on Registration Day, August 15) 2011-12

• Study Hall Guidelines 11-12
• 11-12 BVSD-Summit Parent-Student Sign-off form
• Student Handbook 11-12 (you'll need to read this before signing the Sign-off form above)
• Directory Instructions  (not a form, but gives instructions for new directory process)
• Medication Order and Release - Permission Form (only needed if your child requires medication at school)
