We would like to encourage you to participate in the Birthday Book Program at Superior Elementary School. The purpose of this program is two-fold: first, to celebrate your child's birthday by have a book presented to the library in his or her honor and second, to add new titles to our school library.

If you would like to celebrate your child's birthday in this manner, print/fill out the form below and return it to the library.

Your child will choose a book from our collection of new books and receive a certificate at the time of presentation. All books will have a special gift plate with the child's name and birth date. When the book is processed, your child will be the first to check out the book from the library.

Children will be presented their book during their birthday month. If your child has a summer birthday, we will arrange a special time to present their book.

You may prefer to purchase a hardcover book yourself and bring it to the library. However, plese remember the book must be pre-approved by Mrs. Dirks (pat.dirks@bvsd.org) as appropriate for the Superior Library collection.

Please try to give us 3-4 weeks advance time so we can select and process books. The cost is $15.00, payable to SES (unless you purchase a pre-approved book). Please attach a check when you submit your form.
